McLaren Physician Partners, McLaren Health Care, is seeking a Director of Care Coordination to join the team and support the organization.

JOB SUMMARY

The position is responsible for providing the overall leadership in conjunction with MPP Senior leadership team in the development, implementation and management of Disease/Population Health Management leading to improved patient outcomes.  The Director will serve as a leader in driving clinical care transformation, delivering a comprehensive interdisciplinary approach to achieve care excellence throughout the patient care continuum that measurably improves quality, patient care experiences, and reduces healthcare costs in order to achieve top decile clinical performance and care delivery creating a high performing physician network..

REQUIRED QUALIFICATIONS

  • RN, BSN
  • Seven years of leadership experience in a health plan like environment including Utilization Management, case management, and disease management and population health.

PREFERRED QUALIFICATIONS

  • Masters degree in Nursing, Business or Health Care Administration
  • Ten years of leadership experience in a health plan like environment including Utilization Management, case Management, Disease Management and Population Health.
  • Three years of an ambulatory medicine experience including clinical patient care, patient education, coding and office workflow.
