Description

Role and Responsibility:

§   Develop proactive process and quality control model to effectively identify key gaps and variances across Care4U

§   Utilize process controls to achieve operational excellence in all areas within the Care4U model

§   Actively work with Population Health and Market leadership in closing variances of operational procedures as they relate to the Care4U model

§   Support the development of aligned, collaborative core infrastructure to ensure organization wide alignment with Care4U model

§   Utilize process improvement frameworks to drive continuous improvement across the Care4U model

§   Support process changes that result from market level regulatory/compliance requirements to ensure alignment with Care4U model and minimize unintended model variance

§   Implement systematic models of change through PDCA to coordinate pilots and other evidence based models that impact the Care4U model

§   Provide mentorship and training to Care4U staff and leaders on process improvement and process quality

§   Proactively build and manage various relationships in order to identify process improvement opportunities across Care4U

§   Work collaboratively with population health analytics to define and utilize outcome measurements in defining success

§   Design future state operation models that address opportunities uncovered

§   Partner as needed with the Process Excellence team to support in the implementation of process improvement opportunities

§   Support change management initiatives that impact the Care4U model

Requirements

Education / Experience:

§   Bachelor’s degree in nursing, engineering, business management or related field, or equivalent years of relevant work experience is required

§   A minimum of five (5) years of experience related to the responsibilities of this position is required

§   Demonstrated change management and continuous improvement leadership knowledge is required

§   Previous experience with quality control and/or process standards is required

§   Proficient use of Microsoft Office Suite, to include Word, Excel and PowerPoint; knowledge of  Visio, with a strong willingness to learn

Licensure / Certification:

§   Lean certification is preferred

§   Project Management Certification is preferred
